1. Recruit Account Signature Card - Complete Recruit Account Signature Card
Complete the signature card. You will see areas highlighted in grey. Please fill out all highlighted areas. If there will be a second person on the account (joint signer), that person will need to fill out the Joint Account Holder portion of the Signature Card. If there is no joint signer, leave that section blank.
2. Direct Deposit Form - ARMY | AIR FORCE | NAVY | MARINE CORPS | COAST GUARD
Complete blocks A, B & C on the Direct Deposit Form, and print your name in the Depositor Account Title block. This form will be sent back to you with your Account number in block E.
Please be sure to take this form with you to Basic Training.
3. Fax the signature card, direct deposit form and a copy of front and back of a state or federal issued photo ID, and (if necessary) a copy of the joint account holder's photo Id.
